Shady Grove Fertility Center has once again donated a free treatment cycle, including medication, up to a $10,000 value, to be given away at the Race for the Family 5K on Sunday, September 26. This is the approximate cost of an IVF cycle, but may be put towards any treatment at Shady Grove Fertility.

RAFFLE GUIDELINES

Winner does not have to be present to win.

• One entry will be granted for a donation of at least $25.
• Each donation of $25 will count as one entry into the drawing. Up to 5 entries per sponsor.
• Limit 5 entries per family.
• Prize may be transferred to one person.
• Prize may not be sold or exchanges for any profit. Any indication that this prize has been sold, the prize will be forfeited and no refund will be given.
• The winner of the free treatment cycle is granting permission to share his or her story, name, and image with Shady Grove Fertility and the Cade Foundation for public relations. Failure to do so will result in forfeiture of the prize.

I have read the guidelines and understand that, should I win subsidized treatment with Shady Grove Fertility, I grant permission for my name, story and image to be used for public relations purposes. If I decide to not participate in telling my story, or sharing my name and image, I forfeit the prizes awarded.
